Avancerad äggstockscancer står emot checkpointhämmare eftersom tumören oftast består av en dominant immunhämmande tumörmikromiljö och inte förmår locka till sig T-mördarceller, kritiska immunceller som krävs för att checkpointhämmare ska fungera. Responsfrekvensen har legat kvar på omkring åtta procent i flera stora studier. Ungefär tre av fyra patienter diagnostiseras i stadium III eller IV, och femårsöverlevnaden vid långt framskriden sjukdom ligger mellan 15 och 40 procent. BioInvents läkemedelskandidat BI-1808 är designad för att överkomma denna hämning och få immunförsvaret att attackera tumören.
Riktar in sig på de hämmande cellerna
BI-1808 riktar in sig på TNFR2, en receptor som överuttrycks på immunhämmande celler i tumören. BI-1808 fungerar genom att selektivt eliminera immunhämmande Treg-celler samt omprogrammera immunhämmande så kallade myeloida celler i tumören för att locka till sig mördar-T-celler. Rekryterade mördar-T-cellers effektivitet och uthållighet att bekämpa cancellerna kan förstärkas och upprätthållas genom sambehandling med pembrolizumab. Data från fas I/IIa-studien KEYNOTE-D20, som presenterades på ASCO i maj, visade en objektiv responsfrekvens på 24 procent och en sjukdomskontroll på 56 procent hos 25 kraftigt förbehandlade patienter med äggstockscancer. Den progressionsfria överlevnaden (median) ligger för närvarande på 10,3 månader, med flera pågående responser.
Betydelsen av resultaten framträder tydligast i jämförelse med KEYNOTE-100, den historiska referensstudien för pembrolizumab som monoterapi vid avancerad äggstockscancer, vilken gav en responsfrekvens på åtta procent. KEYNOTE-D20, som genomförs inom ramen för BioInvents kliniska samarbetsavtal med MSD, nådde 24 procent i en mer förbehandlad population, men med samma checkpointhämmare i kombination med BI-1808.
En indikation som visat sig svårlöst
Denna skillnad väger tungt, inte minst eftersom indikationen har varit svår att bemästra. Det senaste försöket med kemoterapifri immunterapi på detta område, nemvaleukin alfa, lades ned i mars 2025 efter att ha missat överlevnadsmålet i en fas III-studie. Andra nya framsteg kräver samtidigt antingen veckovis behandling med paclitaxel eller en specifik biomarkör.
Bland ett tjugotal anti-TNFR2-antikroppar i preklinisk eller klinisk utveckling är BI-1808 för närvarande längst framskriden i ett PD-1-kombinationsprogram, med de mest mogna kliniska resultaten inom äggstockscancer.
En responsfrekvens på 83 procent inom lymfom
BioInvents andra antikropp, BI-1206, riktar in sig på FcγRIIB, en receptor som tumörceller använder för att undkomma rituximab-medierad celldöd vid behandling av non-Hodgkins lymfom (NHL). På EHA i juni uppvisade trippelkombinationen med rituximab och akalabrutinib en total responsfrekvens på 83 procent vid recidiverande eller refraktärt NHL. Andelen kompletta responser översteg flera godkända behandlingsregimer och visade en markant bättre tolerabilitet. Fullständiga fas IIa-data från armen väntas före årsskiftet. Dessa kompletteras av de första resultaten från BI-1206 i första linjens icke-småcellig lungcancer (NSCLC) och uvealt melanom, en presentation på ESMO 2026 för BI-1808 vid platinumbeständig äggstockscancer samt data från expansionskohorten i KEYNOTE-D20.
Resultat som presenterades på EHA i juni stärkte den bilden ytterligare. Vid kutant T-cellslymfom, en sällsynt och ofta behandlingsresistent form av hudlymfom, gav BI-1808 en responsfrekvens på 40 procent som monoterapi i den pågående fas I/IIa-studien, med en komplett respons som fortfarande pågår efter två år. I kombination med pembrolizumab låg responsfrekvensen på 50 procent.

Vi kontaktade vd Martin Welschof för att diskutera den senaste utvecklingen och hans syn på kommande milstolpar.
The expansion cohort in KEYNOTE-D20 focuses on high-grade serous and clear cell ovarian cancer. What do those data need to show for you to move toward a registration-enabling study, and where does initiation of the Part C triplet (BI-1808 + pembrolizumab + paclitaxel) stand today?
– The key objective for the expansion cohort is to confirm and further strengthen the efficacy signal we have already observed with BI-1808 and pembrolizumab across both high-grade serous and clear cell ovarian cancer. We are looking not only at response rates, but also at durability of response, progression-free survival and consistency across patient subgroups. If the data continue to demonstrate clinically meaningful activity and a favorable risk-benefit profile in this heavily pretreated setting, we believe that would provide a strong foundation for discussions around a registration-enabling path. The encouraging interim results presented to date have already shown activity across both histologies, which is particularly important for us.
– With regard to Part C, which combines BI-1808, pembrolizumab and paclitaxel, we see this as an opportunity to explore whether adding chemotherapy can further enhance outcomes while maintaining manageable tolerability. Site activation and study execution are progressing, and we continue to work closely with investigators to advance enrolment according to plan. The triplet arm is an important component of our broader development strategy in ovarian cancer.
FDA Fast Track designation was granted for BI-1808 in ovarian cancer in August. In concrete terms, how does that change your regulatory dialogue from here?
– Fast Track designation is an important validation of both the unmet medical need in platinum-resistant ovarian cancer and the clinical data generated so far. Practically speaking, it allows for more frequent interactions with the FDA and creates opportunities for earlier alignment on development plans, clinical endpoints and the overall regulatory strategy. It can also facilitate a more efficient review process in the future, including the potential for rolling review should the program ultimately support a marketing application.
– For us, the most important benefit is the ability to engage regulators earlier and more frequently as we define the optimal path forward. Our goal is to ensure that we generate the data required for patients, physicians and regulators in the most efficient way possible.
For BI-1206 in NHL, the triplet delivered an 83% response rate with a favorable safety profile. What does the remaining phase IIa data need to show, and where do you stand relative to recently approved regimens before the program is ready for a pivotal study?
– The data generated so far are very encouraging and support our belief that BI-1206 has the potential to address an important need in relapsed or refractory NHL. As we evaluate the remaining Phase IIa data, the areas we will focus on are durability of response, depth of response, consistency across patient populations and continued confirmation of the favorable safety profile.
– The treatment landscape in NHL has evolved significantly in recent years, and any future pivotal program must be positioned against current standards of care rather than historical benchmarks. Therefore, not only by us but also by any potential partner, the assessments are not based solely on response rates but also analyzing the totality of the data package, including efficacy, durability, tolerability, practicality and potential differentiation.
You have signaled that resolving the financing is a priority for the second half of 2026. What paths are you most actively working on?
– Our priority is to ensure that we have the resources necessary to continue advancing our most promising programs while preserving long-term shareholder value. We are evaluating a range of financing alternatives, including strategic partnerships, business development transactions, non-dilutive opportunities and traditional capital market solutions.
– The strength of our clinical portfolio puts us in a constructive position in those discussions. Our objective is not simply to secure capital, but to identify the financing structure that best supports the company’s long-term growth and maximizes the value of our assets.
Finally, when you look back at 2026 a year from now, what needs to have happened for it to have been a successful year – and what are the main goals you are already setting for 2027?
– If I look back one year from now, I will consider 2026 a successful year if we have delivered additional clinical validation for both BI-1808 and BI-1206 in solid tumors, established a clear regulatory path for our lead programs, and secured the financial flexibility needed to execute our strategy.
– More specifically, we want to further strengthen the ovarian cancer opportunity for BI-1808, continue demonstrating the value of BI-1206 in solid tumors, and advance discussions that can accelerate developments and unlock the full potential of our pipeline.
– Looking ahead to 2027, our focus would be on translating those achievements into the next phase of value creation and continuing to position BioInvent as a leader in next-generation cancer immunotherapy. Ultimately, success will be measured by how effectively we move our most promising therapies closer to patients who need them.
