SynAct om fas III-planerna efter nya ADVANCE-data

SynAct Pharma har presenterat en post hoc-analys av fas IIb-studien ADVANCE med resomelagon inom reumatoid artrit. Dessa nya data klargör de tidigare placeboresponserna och stärker läkemedelskandidatens terapeutiska profil. Bolaget förbereder nu diskussioner med FDA om utformningen av en fas III-studie under fjärde kvartalet 2026. BioStock kontaktade CSO Thomas Jonassen och CBO Mads Bjerregaard för en kommentar.

I stället för att enbart dämpa immunsystemet syftar SynAct Pharma till att häva svår inflammation genom ett angreppssätt kallat pro-resolution. Bolagets ledande kandidat, resomelagon (AP1189), är utformad för att uppnå detta genom selektiv aktivering av melanokortinsystemet.

Den fördjupade genomgången av toplineedata från ADVANCE som rapporterades i juni utlöstes av en ovanligt hög klinisk respons i kontrollgruppen som fick placebo i kombination med metotrexat. Beslutet att titta närmare på juni-datan tog avstamp i den oväntat höga responsen hos patienterna som fick placebo. Genom att granska siffrorna i detalj har bolaget nu fått en tydligare bild av hur effektivt resomelagon faktiskt är.

Förklaringen bakom placeboresponsen

Analysen visade att ungefär en av fem patienter i ADVANCE bedömdes befinna sig i ett sjukdomsskov vid inklusionen, vilket återspeglade en tillfällig topp i inflammationen. Denna undergrupp stod för den stora majoriteten av variationen i behandlingsrespons i placeboarmen.

Bland de övriga fyra av fem patienterna, med mer stabila nivåer av högkänsligt CRP vid baslinjen, uppnådde 79 procent ett ACR20-svar med resomelagon 40 mg dagligen, jämfört med 55 procent i placebogruppen. ACR50-responsen uppgick till 38 procent i den aktiva gruppen mot 30 procent för placebo, vilket stämmer väl överens med de positiva fynden från den tidigare fas IIb-studien EXPAND.

Anpassad studiedesign och regulatoriska steg

Med dessa insikter anpassar nu bolaget det kommande fas III-programmet för att minska påverkan av instabilitet vid baslinjen. Den föreslagna studiedesignen, tillsammans med ett stödjande toxikologiprogram, ska lämnas in till FDA i början av fjärde kvartalet för diskussion vid ett Type C-möte. Återkoppling väntas före årets slut eller i början av 2027.

CSO Thomas Jonassen beskriver fynden som ett tydligare underlag för fas III-utformningen. Målsättningen är att säkerställa mer stabil inflammatorisk aktivitet inför randomisering, med bibehållet fokus på patienter med tidig RA. Han framhåller att en förlängning av behandlingen till 26 veckor ger möjlighet att bedöma om den starka ACR20-respons som observerades vid vecka 12 utvecklas till en mer uttalad ACR50-respons.

Q&A med CSO och CBO

BioStock kontaktade CSO Thomas Jonassen och CBO Mads Bjerregaard för att diskutera den nya ADVANCE-datan, den uppdaterade studiedesignen och de kommande FDA-förberedelserna.

Thomas, the ADVANCE study’s primary endpoint didn’t reach statistical significance back in June. What does this new post-hoc analysis add to that picture?

– The primary endpoint, change in DAS28-CRP, reached a reduction from baseline of 2.0 in the resomelagon 40 mg group versus 1.8 reduction in the placebo treated control group which was not enough to reach statistical significance. In our EXPAND study, the placebo control group reached 1.2 points difference and therefore markedly different to what we saw in the ADVANCE study. Similar observations were made on key secondary endpoint ACR20 with placebo group response reaching 60% which is the expected ceiling for MTX therapy.

– What this analysis does is to explain why. When we looked closely at the placebo arm, we found that around one in five patients were enrolled during an active flare-up — a temporary spike in disease activity rather than their normal baseline. That group swings hard toward improvement on its own, treatment or not, simply because they’re regressing back to their normal level. It’s a well-known statistical effect, and it diluted the signal in the full dataset. When we isolate roughly four in five patients with a more stable baseline, we see the true treatment effect which clearly and lines up with what we saw in the EXPAND study.

Can you put some numbers on that for readers?

– In the more stable patient group, the true treatment effect of resomelagon 40 mg produced an ACR20 response of 79% versus 55% on placebo, ACR50 of 38% versus 30%, and DAS28-CRP improved by 2.0 points versus 1.5 points on placebo. That’s consistent with the levels we reported in the EXPAND study, in patients with hsCRP above the normal range. The true treatment effects derived from the ADVANCE study accounting for baseline instability is confirming the observations seen in the EXPAND study.

How does this change the Phase 3 program?

– It gives us insights for a sharper design and the tools to address the baseline instability. We’re also extending treatment duration to 26 weeks, so we can see whether the strong ACR20 response we saw at week 12 develops into a more pronounced ACR50 response over time. We’re taking that design, together with the development plan for the supporting toxicology package, into a Type C meeting with the FDA in Q4.

Mads, what does this mean for SynAct’s partnering discussions in RA and autoimmune disease?

– It strengthens our hand. Partners want to see a coherent story – a clear mechanism, a repeatable effect across two studies, and a credible path through Phase 3. This analysis gives us all three. We’re evaluating the full range of strategic alternatives to maximize shareholder value –  global or regional licensing agreements, an outright sale of resomelagon, or a potential sale of SynAct itself. Those remain a priority for the company, and this data package is exactly what strengthens our position in those discussions.

How should investors think about the equity story from here?

– The near-term catalysts are well defined: FDA feedback on the Phase 3 design, expected by year-end or early 2027; the first commercial agreement in Brazil for our dengue program; and early data from RESOVIR-2, subject to local regulatory approval of the protocol amendment. In parallel, we continue to evaluate strategic partnerships and transactions across the portfolio – from licensing to a potential sale of resomelagon or of the company – so investors are looking at multiple, independent paths to value creation, not a single binary outcome.

What should the market watch over the next twelve months?

– A few things: the FDA’s feedback on Phase 3 trial design; the first commercial agreement in Brazil for dengue and Part 1 data from RESOVIR-2; full Phase 2 data from RESOVIR-2 and from our RESPIRE study in respiratory viral infections; and progress on strategic partnerships and transactions. Taken together, those milestones are what turn this data update into a fully de-risked, transactable RA program – alongside a broader host-directed pipeline.
